Obesity presents one of the greatest threats to healthy aging.  In 2009, an estimated 200 billion U.S. dollars was spent to treat patients with morbid obesity.  Over half of the U.S. population is estimated to be overweight, and about one third are morbidly obese.  They have even come out with a new category called super morbidly obese!  Obesity as a disease of Western society is expected to continue to increase markedly over the next two decades.

 

Obesity is defined as an increase in the number and size of adipocytes in fat tissue.

  • Overweight is a Body Mass Index (BMI) of 25 – 29.9
  • Obesity is a BMI of 30 -46.9
  • Super-Obesity is a BMI of 47 or more

 

Obesity if classified as a disease, as the overall mortality rate of the morbidly obese population has been demonstrated to be approximately double that of the normal weight population.  More staggering is the realization that in  25 – 34 year old males the same mortality rate is increased 12 fold!  This increase in mortality is explained by the medical conditions or co-morbid factors frequently associated with morbid obesity.

 

Obesity is expected, within a few years, to become the number one preventable cause of death in the U.S. (currently it is ranked second)

So, what can you do about obesity?

I treat many patients for obesity.  I prefer to help my patients to get healthy and to have weight loss as a side effect of that.

 

Obesity rarely results from one single factor.  It has many causes, including nutrition, activity level, hormones, stress, lifestyle, sleep, and genetics.  Some researchers estimate that up to 70% of the variability in weight among humans can be explained by genetic influences, but it is extremely unlikely that changes in human genes account for the recent change in obesity prevalence.  The good news is that most of the variables contributing to obesity can be prevented, limited, or managed through lifestyle choices.

 

Adherence to a specific diet has been shown to be the most important factor in achieving weight reduction.  I like to use hCG injections with the very low calorie diet to basically reset the hypothalamus so that the body starts reacting appropriately to calories in versus energy out.

 

For long term maintenance of a healthy weight, a patient needs aggressive intervention that addresses all of the contributing factors – diet, exercise, hormone optimization, nutritional supplementation, diagnosis and treatment of any sleep apnea or other sleep issues, lifestyle improvements, and stress management.  Moreover, lifelong success depends upon a personal commitment and self-discipline.  The important thing to remember is that you need to get healthy to lose weight.

Myth #4: Exercise Burns Most of Our Calories

Not even close!

 

Anyone who has been bullied by an overzealous trainer or other “expert” should enjoy reading this.   Hip hip hooray!  There is a smarter way to increase your metabolism other than absolutely killing yourself working out.  Working out is just a part of the puzzle.

Even the most fanatical fitness nuts burn no more than 30 percent of their daily calories at the gym. Most of your calories burn at a constant simmer, fueling the automated processes that keep you alive—that is, your basal metabolism, says Warren Willey, D.O., author of Better Than Steroids. If you want to burn fuel, hit the gas in your everyday activities.  The little things can make a big difference in burning calories and losing weight.

“Some 60 to 70 percent of our total caloric expenditure goes toward normal bodily functions,” says Howell. This includes replacing old tissue, transporting oxygen, mending minor shaving wounds, and so on. For men, these processes require about 11 calories per pound of body weight a day, so a 200-pound man will incinerate 2,200 calories a day—even if he sat in front of the TV all day.

And then there are the calories you lose to N.E.A.T., or Non-Exercise Activity Thermo-genesis.   N.E.A.T. consists of the countless daily motions you make outside the gym—the calories you burn while making breakfast, playing Nerf football in the office, or chasing the bus. Brandon Alderman, Ph.D., director of the exercise psychophysiology lab at Rutgers University, says emerging evidence suggests that “a conscious effort to spend more time on your feet might net a greater calorie burn than 30 minutes of daily exercise.”

Your move:

Take frequent breaks from your desk (and couch) to move your body and burn bonus calories.   Start your day with some movement and punctuate your day with more steps and movement, for instance parking a little further from the building than you usually would or taking the stairs instead of the elevator.

Myth #2: All Calories Are Created Equal

Not exactly

Your body’s energy  comes from three sources: protein, carbohydrates, and fat. “They’re handled by the body differently,” says Alan Aragon, M.S., a Men’s Health nutrition advisor. So that old “calories in, calories out” formula can be misleading, he says. “Carbohydrates, protein, and fat have different effects on the equation.”

Example: For every 100 carbohydrate calories you consume, your body expends 5 to 10 in digestion. With fats, you expend slightly less (although thin people seem to break down more fat than heavy people do). The calorie-burn champion is protein: For every 100 protein calories you consume, your body needs 20 to 30 for digestion, Buchowski says. Carbohydrates and fat give up their calories easily: They’re built to supply quick energy. In effect, carbs and fat yield more usable energy than protein does.

Beyond that, carbs spike your blood sugar, which then spikes your insulin, which then drops your blood sugar and makes you crave more carbs.  That is a vicious cycle that many people chase all day long starting with that high-carb bowl or cereal or pancake breakfast.

Your goal should be 21 grams of protein in a meal.  Less is fine for a snack.

Your move:

If you want to lose weight, make protein a priority at every meal.   Adding them to snacks—especially before you exercise—can help too.  Protein can be from animals, but can also be vegetarian or vegan.    Proteins are made up of amino acids, and when digested return to their amino acid form.  There are certain amino acids that are required to be consumed because our bodies can’t make them.   This gets trickier for vegetarians and especially for vegans, so do your homework if you have any doubts about this in your own diet.

Calorie Myth #1: Calories Fuel Our Bodies

Actually, they don’t

A calorie is just a unit of measurement for heat; in the early 19th century, it was used to explain the theory of heat conservation and steam engines. The term calorie entered the food world around 1890, when the USDA used the term in a report on nutrition. Specifically, a calorie was defined as the unit of heat required to raise 1 gram of water 1 degree Celsius.   Does this remind you of science class?

To apply this concept to foods like that plate of spaghetti, scientists used to set food on fire (really!) and then gauge how well the flaming sample warmed a water bath. The warmer the water, the more calories the food contained. (Today, a food’s calorie count is estimated from its carbohydrate, protein, and fat content.) In the calorie’s leap to nutrition, its definition evolved. The calorie we now see listed on nutrition labels is the amount of heat required to raise 1 kilogram of water by 1 degree Celsius.

Here’s the problem: Your body isn’t a steam engine. Instead of heat, it runs on chemical energy, fueled by the oxidation of carbohydrates, fat, and protein that occurs in your cells’ mitochondria. “You could say mitochondria are like small power plants,” says Maciej Buchowski, Ph.D., a research professor of medicine at Vanderbilt University medical center. “Instead of one central plant, you have several billion, so it’s more efficient.”

For your mitochondria to work properly, you must have certain critical co-factors.  If you do have everything critical for optimal mitochondrial energy production, then you will get 36 units of concentrated energy out for a standard input.  If you are missing certain critical co-factors, B-12 as an example, then your mitochondria will only produce 2 units of energy – plus byproducts like lactic acid or pyruvic acid that then make you feel like you have been exercising way too much because they make your muscles feel sore.

It is really too bad when you are overfed and undernourished, and because of that, you are fat and too tired to move much!

Your move:

Track carbohydrates, fats, and protein—not just calories—when you’re evaluating foods, and pay attention to your nutrients so that you get maximum energy production for all that  food in.

How Much Weight Can I Expect to Lose on the Austin hCG Weight Loss Program?
